First Visit

When you come for your appointment please remember to obtain and bring the following:

• Driver’s License or a valid ID
• Insurance information
• Referral Letter (if required)
• Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
• List of medications (if any)

Primera Visita

Cuando acuda a su cita, recuerde obtener y traer lo siguiente:

• Licencia de conducir o una identificación válida
• Información del seguro
• Carta de remisión (si se requiere)
• Informes, radiografías, resonancias magnéticas, tomografías, etc., y cualquier otra información relevante
• Lista de medicamentos (si los hay)
